Group Life Insurance

Premium on first
$25,000 paid by employer, this amount included as taxable benefit,
balance of premium paid by employee
- Coverage is 3
times annual salary to a maximum of $1 million, proof of insurability
is required for amounts of life insurance in excess of the no
evidence maximum of $900,000.
- Group Life
Insurance terminates at age 71, however if an employee terminates
employment prior to this age, he/she is eligible to apply for
individual policy coverage within 31 days after last day worked.

Employees: Preventative treatment 100%, routine
treatment 80%, major and orthodontic treatment 50%, Maximum the
plan will pay in any one calendar year is $1,500 for each insured
family member.

Employees: Preventative treatment 50%, routine treatment 40%,
major and orthodontic treatment 25%, Maximum the plan will pay
in any one calendar year is $750 for each insured family member.

This coverage
terminates when the employee terminates employment.

Basic
Accidental Death & Dismemberment (AD&D)

Provides principal
sum of $50,000 for full-time employees upon death or accident and
$25,000 for eligible part-time employees where various accidents are
a percentage of principal sum; additional repatriation,
rehabilitation, occupational training and medical (hospital expense benefits).
- AD&D
eligibility for coverage up to age 70, however should an employee
terminate employment prior to this age, he/she is eligible to apply
for individual policy within 60 days after last day worked.

optional or voluntary basis to employees and their families.
Coverage (Principal Sum) may be purchased in increments of $10,000
from a minimum of $50,000 to a maximum of $250,000.
